Store calendar data by GUID

Same as we did for principals. Can be largely transparent to clients; calendar-home should still point to the same URLs but we should store data by GUID under the covers.

This gives us some valuable uniqueness when the same short name exists in multiple records in the directory (or when bound to >1 directory).

It also solves #111, I think, as well as the case where a short name is changed.

Fixed in r2220 on trunk and r2221 for 1.2.
